Symptoms of Endometriosis
1. Dysmenorrhea: Patients complain that there was no pain during menstruation in the past, but they began to experience dysmenorrhea from a certain period. It can occur before, during, and after menstruation. Some menstrual cramps are severe and unbearable, requiring bed rest or medication to relieve pain. Pain often worsens with the Menstrual cycle. The level of estrogen continues to increase, causing proliferation and swelling of the ectopic endometrium, bleeding under the influence of pregnancy hormones, stimulating local tissues, and causing pain. If it is an internal Endometriosis, it can promote uterine muscle contracture, and dysmenorrhea will certainly be more significant. There are no cases of bleeding in ectopic tissues, and their dysmenorrhea may be caused by vascular congestion. After menstruation, the ectopic endometrium gradually atrophies and dysmenorrhea disappears. Clinically, there is significant endometriosis, but about 25% of cases have painless menstruation. The psychological state of women can also affect pain perception.
2. Excessive menstruation: internal Endometriosis, the amount of menstruation tends to increase, and prolonged menstruation may be caused by increased endometrium, but it is often accompanied by ovarian dysfunction.
3. Infertility: Patients with endometriosis often have infertility. The causal relationship between infertility and endometriosis is still controversial. Pelvic endometriosis often causes adhesions around the fallopian tubes, affecting oocyte pickup and lumen blockage. Ovarian lesions affect the normal progression of ovulation, leading to infertility. However, some people also believe that when long-term infertility and menstruation do not stop, there is a chance of endometriosis pregnancy, and the ectopic endometrium will be suppressed and atrophied.
4. Dyspareunia: Endometriosis occurs in the uterine rectal fossa and the vaginal rectal septum, which causes swelling of surrounding tissues and affects sexual life, and the discomfort in premenstrual period increases.
5. Stool distension: It usually occurs in the pre menstrual period or after menstruation. The patient feels pain when the stool passes through the rectum, but does not feel this feeling at other times. It is a typical symptom of Endometriosis in the uterine rectal fossa and near the rectum. Occasionally, I saw ectopic endometrium reaching deep into the rectal mucosa, causing rectal bleeding during menstruation. People with endometriotic lesions that surround the rectum and form stenosis have symptoms of internal urgency, followed by congestion, similar to cancer.
