What is the reason why women do not ovulate? Can women still conceive without ovulation
Female anovulation is a chronic disease, and anovulation is the cause of infertility in at least one-third of cases. Female anovulation usually manifests as irregular menstruation, with less time per year than normal, or even complete cessation. The reasons for not ovulating can be divided into the reasons for inducing egg release therapy and the reasons for not being able to receive such treatment.
What are the causes of female anovulation?
1、 Hypothalamic or pituitary causes
1. Functional anovulation
In some cases, functional anovulation is attributed to hypothalamic suppression caused by deep pressure, excessive exercise, or severe malnutrition. In anovulation of the hypothalamus, the secretion of gonadotropin releasing hormone in the hypothalamus is affected, resulting in a secondary effect on the pituitary gland due to the failure to stimulate gonadotropin release. In this case, the basal levels of gonadotropins and estrogen are normal or only slightly reduced, but the pulsatile secretion of gonadotropins required for physiological effects is not significant. These women responded well to the administration of gonadotropin releasing hormone.
2. Sean syndrome
Sheen syndrome is a condition accompanied by a large number of infarcts in the anterior pituitary gland, such as after intense bleeding, usually after childbirth or trauma.
3. Kalman syndrome
Kalman syndrome is a congenital hypothalamic encephalopathy in which there is no occurrence of gonadotropin releasing hormone neural elements. Kalman syndrome is usually associated with insufficient or insufficient olfactory perception.
4. Brain radiation
Children who have received radiation therapy for intracranial tumors or have leukemia may cause damage to the hypothalamic and pituitary regions, leading to failure of gonadotropin secretion.
5. Hyperprolactinemia
In this case, there is a high level of hormone prolactin, which suppresses gonadotropins by producing negative feedback on its secretion, typically resulting in pituitary tumors called microadenomas.
6. Ovarian diseases
The most common ovarian cause of anovulation is polycystic ovary syndrome, which accounts for approximately 70% of all cases of anovulatory infertility. This is almost always found in young adult women with hirsutism, acne, or long-term irregular menstrual cycles. Many people are overweight, and insulin resistance seems to play a major role in insufficient ovulation.
2、 Ovarian failure
These diseases have primary ovarian causes that do not release eggs and cannot be corrected through ovulation induction. The ovaries themselves may not develop properly, leading to failed egg release. Ovarian failure may occur due to chromosomal abnormalities, surgical or ovarian resection, immune damage to the ovaries themselves, ovarian damage to the abdominal and pelvic areas through chemotherapy or radiotherapy, and idiopathic ovarian failure.
