Does endometriosis affect fertility?
Endometriosis is one of the most common causes of infertility, accounting for 50% of the infertile population. The mechanism by which endometriosis affects infertility is mainly due to its destruction of the pelvic and fallopian tube structure, leading to infertility. In addition, endometriosis affects the pelvic environment and interferes with embryo growth and cultivation. Research has shown that infertility and miscarriage occur. Therefore, patients with endometriosis are also concerned about infertility.
The causes of endometriosis affecting fertility
1. Ovulation process: Some patients experience luteinization of the follicles when they are ready to ovulate, but the follicles are not expelled in the end. However, in basic body temperature tests and ovulation test strips, ovulation is the same as normal ovulation. The hormone secretion of the ovaries is also similar to normal ovulation. Under ultrasound, it can be seen that the ovaries are gradually maturing, but atrophy and luteinization occur in situ.
2. Egg transport: In patients with endometriosis, some may experience pelvic adhesions, tubal adhesions, or even obstruction. The fallopian tube is malfunctioning and cannot transport sperm to the predetermined location to meet the egg.
3. Fertilization process: Patients with endometriosis, especially those with dysmenorrhea, have a poor intraperitoneal environment and a large number of inflammatory factors that can harm sperm and fertilized eggs, engulf sperm and fertilized eggs.
4. Implantation stage: There is now a theory of in situ endometrial determinism. The reason why a person may develop endometriosis is mainly due to problems with the endometrium itself, which can affect the implantation of fertilized eggs.
Treatment of endometriosis
1. Drug therapy
Using drugs to counteract or suppress periodic endocrine stimulation of the ovaries. Initially, the use of testosterone as a male hormone had significant side effects and limited efficacy, so it gradually gave up. Afterwards, it gradually developed into pseudo pregnancy therapy and pseudo amenorrhea therapy.
(1) Pseudo pregnancy therapy
The use of pregnancy hormone drugs in larger doses for long-term use, cessation of menstruation, and the occurrence of pregnancy like reactions in the endometrium and ectopic endometrium under the action of drugs, is also known as pseudo pregnancy therapy. There are many drugs used for this therapy, and they are still in development. Oral administration mainly includes progesterone, praveratrol, and nemetone, while intramuscular injection has its own acid progesterone. This treatment lasts for at least 6 months, stopping the activity of the ectopic endometrium and eventually atrophy, resulting in therapeutic effects.
